NEW PATIENTS:  

We are not currently taking new patients as our practice is full.  Check back in 2027 for updates.  Thank you.
 
Our office requires referrals, even if your insurance does not.  Please have your current rheumatologist and/or your referring doctor fax over chart notes, labs, your insurance card and contact information and we will call you once you we have reviewed it. The wait time for a new patient visit is usually 8-12 weeks.  We do have a cancellation list as well.
 
If you need to cancel or reschedule your appointment, please call us at least two days prior to your appointmentn date. There will be a $50 charge for late cancelations or no-shows for new patient appointments.

We cannot see you as a patient if you have AHCCCS or Medicaid (primary or secondary).
